Studying Ways to Lower Blood Pressure and Prevent Another Stroke
Stroke survivors are at high risk of having another stroke; one in four will have another stroke. Hypertension is a leading cause of stroke. The purpose of this study is to help patients who have had a stroke to control and lower their blood pressure to prevent another stroke. We will compare two ways to do this: clinic visits with health education and personalized telehealth care. This study will help us find out which method works best for different patients. We are looking for volunteers who have had a stroke and have been discharged home from acute care or acute inpatient rehabilitation facility.
